1/7/2024 0 Comments Cardizem dripMany nurses are apprehensive about to titrate a Cardizem drip.It may be used alone or in combination with other high blood pressure medications. Cardizem is used to treat hypertension (high blood pressure). It works by the muscles of your heart and blood vessels. □ Encourage the patient to comply with other heart healthy interventions like weight loss, low sodium diet, smoking cessation and stress management. Cardizem (diltiazem) is a calcium channel blocker.□ Educate the patient to take the medication the same time each day and to report rash, SOB, excessive swelling, severe headache □ Monitor ECG continuously during administration □ Assess for signs of HF and peripheral edema □ Closely monitor BP and HR during titration of diltiazem drips □ Monitor BP and pulse prior to administering Nursing Tip: I usually just set the IV pump for a volume. They are started at 1 mg/min for 6 hours, and then moved down to 0.5 mg/min for 18 hours. It seems that most commonly patients are given a 300 mg bolus of amiodarone initially and then given a continuous drip. ▪️2nd or 3rd degree heart block unless pacemaker is in place Side Effects □ Make sure to administer amioidarone according to your providers prescription. □Should be used cautiously in HF and patients with renal impairment Contraindications □ □There is a higher risk of hypotension in hepatic impairment and geriatric patients ♥️Blocks calcium channels in the myocardium, SA node and AV node ➡️ decreased HR, slows rate of conduction through the □ Precautions ⚠️ ♥️Blocks calcium channels in the blood vessels ➡️ vasodilation of peripheral arterioles and arteries ❣️Reduces ventricular heart rate in cardiac dysrhythmias like A-Fib and SVT How does it work❓ ❣️Causes coronary vasodilation to treat angina I actually enjoy having a patient on a Cardizem drip because I have the autonomy to titrate the drip rate and see an immediate change in their heart rate and condition. It’s given via PO, IV push and IV infusions □ □ Cardizem is one of the more common cardiac meds I give on the telemetry/step-down floors.
